- Continuity: Boylar's insignia indicates he is a captain, yet he is referred to as a lieutenant.
- Continuity: Ilario's cigarette by the pool.
- Continuity: Ilario's position when at the pool with Serling.
- Continuity: In the tank combat sequence, Serling asks, "Do you or do you not have a fucking target?" When a recording of this is later played, the expletive is missing.
- Continuity: Just after the Huey is hit, Ilario is seen hanging out of the left side of the helicopter. In the very next shot from inside the helicopter, Ilario is still inside the helicopter, then falls back as the helicopter spins out of control.
- Continuity: Karen shoots more than 100 rounds from the M-16's 30 round magazine.
- Continuity: The medal hung around Karen's daughter's neck disappears during the flyover.
- Continuity: When Col. Serling is in the motel room looking at a photo of the mother and daughter sitting on a porch swing, the view of the back of the photo shows it is substantially wrinkled. When the front of the photo is shown it is unwrinkled.
- Continuity: When Serling leaves Hershberg's office after their confrontation, he is seen from the inside of the office not carrying anything, but when we see him from outside (immediately after) he is carrying his files.
- Continuity: When taping a conversation, Col. Serling only depresses the "play" button, not the "record" button.
- Continuity: When the tape from Al Bathra is played back at the end, there are numerous changes in dialogue.
- Crew or equipment visible: Reflected in the TV screen in the General's Office
- Errors in geography: The Capitol dome cannot be seen from Bethesda, Maryland.
- Errors in geography: When the two men are talking in what appears to be a park in Washington, D.C., an Austin, Texas, Capital Metro City Bus drives by in the background.
- Factual errors: Even though Monfriez also served in Panama, he was not entitled to a second Combat Infantryman Badge; the CIB is awarded for WWII, Korea, and all conflicts from Vietnam onwards. The only way to earn more than one CIB is to have served in either Korea or WWII.
- Factual errors: Lt. Col. Serling tells Gen. Hershberg that the tank was "hit by a uranium spent projectile" rather than using the correct terminology, "spent uranium projectile."
- Factual errors: The downed helicopter crew that Karen's helo goes in to rescue is referred to as a[UH-60] Blackhawk, when in fact, it is a[UH-1] Huey just like her Medevac chopper.
- Factual errors: The M1A1 used by the 2nd ACR (Armored Cavalry Regiment) of which Col. Serling plays a part fires a 120mm combustible case round. Only the aft end cap comes out of the breech after firing, not the whole casing.
- Factual errors: The Medal of Honor is never placed around the neck of anyone but the recipient of the award.
- Factual errors: We see a locomotive skidding with its wheels locked in a shower of sparks. Locomotive wheels do not lock up when the emergency brakes are applied because, a) it won't stop the train any faster, and b) it would create flat spots on the wheels which are very expensive to repair.
- Incorrectly regarded as goofs: While Dr. Mary E. Walker was the first woman to be awarded the Medal of Honor (in 1866, for bravery as a surgeon), Capt. Walden would have been, as stated, the first woman to be nominated for the medal "for performance under fire".
